Let Us Know What You Think

Do you shop and/or work at the 4th Street Food Coop? Please fill out this short survey about your experience. It will help us identify goals and ideas to adjust, collectively, what we offer and promote (including, but not limited to, the products we carry). We really welcome your thoughts and specific suggestions, even if they may not result in immediate changes. The survey should take less than 10 minutes and will be open until mid-January.

Anyone who shops at the co-op (working members, non-working members, and non-members) is encouraged to answer. Feel free to circulate widely!

Thank you for taking the time to contribute to shaping the coop’s future,

If you have any question[s] related to the survey, please contact: survey AT 4thstreetfoodcoop.org.
